Having used Open Office on Linux and having now changed to Mac, I thought I would give NeoOffice a run. It seems good, except for the database application which crashes when I try to use the wizard to create a form. This may be a java issue but I do not know.
Can anybody out there offer advice/guidance?

Welcome to the NeoOffice forums/fora!
I just tried the form wizard, and it works for me without crashing.
Do you have the most recent patch? (patch 11) If not, you should download it from http://www.planamesa.com/neojava/en/patch.php.
If you are using the latest patch, I would suggest you file a Bug report at Bugzilla (link in one of the boxes to the left). Make sure to set the drop fields for patch and OS version. You should also provide a list of reproducible steps and, if at all possible, a sample file. Attaching the crash report would also be helpful. (Include it as an attached file, don't copy and paste it into the comments field, as that formats it in a way that is hard to read).

No go for me on an iMac core 2 duo. The db fields don't reproduce onto the form; the three fields I set up were represented by a single black line on the data form! |

I work on an iBook, so we may be looking at a PPC vs. Intel bug or issue.
If you have downloaded and installed the latest patch, the thing to do now is to file a bug report. I know there is at least one Intel Mac user who watches Bugzilla pretty carefully and tries to reproduce the reported bugs.
Unless there are other ideas? |

After a little bit of experimentation I can tell you the following:
NeoOffice: All fields in one line
OOo 2.0.3: Crashes after sub-form stage
OOo 2.0.4: Works perfectly
OOo 2.1 : Works perfectly
This is all on Intel. There were problems with wizards in OOo 2.0.3 on Intel which Patrick fixed in Neo 2.0 Beta and which OOo have fixed in 2.0.4 and later.
So, it might be a NeoOffice bug, but its impossible to tell as I can't get to the same stage in OOo 2.0.3 (on which NeoOffice is based). If you can wait until the end of February it should become clear as Neo 2.1 will then be based on OOo 2.1.

I can also confirm the issue does does not persist in OOo 2.1
