I could answer your questions specifically if we knew exactly which font was getting selected as the first fallback. I do not have way to determine that without writing more code as that first fallback font is determined by a Mac OS X function that we call and their code is not accessible to us.
As for why the Arial fallback font is showing on a whole line, I have a theory but it is difficult to test without having the actual font that was being selected as the first fallback font on your machine. Since that font is unknown, I instead wanted to provide a fix that would work for most users now as I was unsure that you can manually identify that font without following Lorinda's steps for each font in the Font Book application.
If you do determine at some point which font appears to be the old fallback font, can you post the "Version", "Location", and "Unique name" fields for that font so that we can see if it is a font that we can match it up to?

I found the fallback font:
Version Version 5.01.3x
Location /Library/Fonts/Times New Roman.ttf
Unique name Monotype:Times New Roman Regular:Version 5.01 (Microsoft)
I do, however, prefer the palatino commas, apostrophes etc. (as they do not default to times new roman) if we can get it working under that font.

I can confirm that I have that same font on my Mac OS X 10.6.3 machine. I even see that the Times New Roman font is getting selected as the fallback font, but none of the Hebrew characters fallback to another font on my machine.
What happens when you select your entire document using the Edit :: Select All menu and then set the font to Times New Roman? Do you see any Arial characters?

OK. Then that makes my fix simpler. I can just use Times New Roman as the first fallback font to look for when using Hebrew. I admit that it does look better than both Raanana and Arial and since most users see Arial as the fallback font, this should be an improvement.
Unless anyone has strong reservations against using Times New Roman as the preferred Hebrew fallback font, I will make the change and post a test patch this weekend.
